The SSS E4 form (or E-4 form) is the Member Data Change Request form that Social Security System (SSS) members need to fill out and submit whenever they want to change or amend their personal information, such as their membership type, name, date of birth, civil status, and contact information.

Let’s say that you got married recently and you want to update your SSS member information in order to replace your maiden name with your new married name. You can do this by submitting a duly accomplished E-4 form to the nearest SSS office along with your marriage certificate.

In many cases, submission of the SSS E4 form is no longer necessary since the SSS member portal already offers members the ability to update their personal information online without visiting the SSS office. SSS members will just need to indicate the specific changes to their membership data and upload a digital copy of a supporting document such as a birth certificate, if necessary.

Nevertheless, there are some instances when the personal submission of the SSS E4 form is required, such as correction of date of birth. Only simple corrections of personal data are allowed on the SSS online portal. Examples are correction of name involving one or two letters, change of gender, and change of civil status from single to married. The SSS strongly encourages members to update their personal data on their My.SSS account instead of submitting the E4 form. That way, congestion at SSS branches is minimized. How to Fill Out the SSS E4 Form

In this section, we’ll teach you how to fill out the SSS E4 form depending on the type of amendment you intend to do.

Get a black pen and write all the required information in capital letters. Always indicate “N/A” if the required data is not applicable.

Personal Data

  • SS Number – Write your 10-digit SSS number. Read this article if you lost or forgot your SSS number.
  • Common Reference Number (if any) – This is completely optional. Write your 12-digit CRN which is printed on your UMID card.
  • Date of Birth – Provide your birth date in MMDDYYYY format. For example, if your birthday is March 4, 1990, write 03041990. Use this tool to get the correct date format.
  • Tax Identification Number (if any) – Include your TIN if you have one. This is optional.
  • Name – Write your last name, first name, middle name and suffix (if any) in that order.
  • Address – Write your complete home address including city/municipality and province.
  • ZIP Code – If you don’t know your ZIP code, refer to this article.
  • Telephone Number – This is optional. Include your area code if you have a telephone number.
  • Mobile/Cellphone Number – Write your 11-digit mobile number.
  • E-mail Address
  • Foreign Address (if applicable) – Indicate your foreign home address if you are residing outside the Philippines.
  • Country – Only fill this out if you have a foreign address.
  • ZIP Code – Only fill this out if you have a foreign address.

Data Change/Correction/Updating

A. Change of Membership Type

Fill out this section if you want to change your membership type to self-employed or non-working spouse. Check the box for your current membership type (employed, voluntary, overseas Filipino worker, non-working spouse, or prior registrant), and then check either “Self-Employed” or “Non-Working Spouse.”

If you selected “Self-Employed,” write down your profession or business, year profession or business started, and monthly earnings. If you selected “Non-Working Spouse” (for prior registrant only), write the SSS number or CRN and monthly income of the working spouse, and then have the working spouse affix his/her signature over his/her name.

B. Correction of Name

If you want to correct your name in your SSS records due to an error in your birth certificate or a legal change of name, you can check any of the following (be sure to fill out both “From” and “To” fields):

  • Last Name
  • First Name
  • Middle Name
  • Prefix (e.g. “de”, “dela”, delos”, “del”, “Ma.” or “Maria”) or Suffix (e.g. Jr., II or III)
  • Simple Error in Spelling of Name (e.g. “i” to “e” or “u” to “o” or vice versa; inclusion/deletion of space and special characters)
  • Due to Re-marriage

C. Correction of Date of Birth

If there has been an error in your date of birth, check this box and accomplish both “From” and “To” fields with the incorrect and correct dates of birth, respectively.

D. Correction of Sex

If there has been an erroneous entry of sex or gender in your birth certificate or in the SSS membership record, check this box and fill out both “From” and “To” fields. For example: Male to Female.

E. Change of Civil Status

Complete this section if you have a change of civil status such as marriage or legal separation. If you are a recently married female member, don’t forget to fill out both “From” and “To” fields in order to request a change of name.

  • Married to Widowed
  • Reversion from Married to Single

F. Updating of Contact Information

This section is redundant as the SSS member portal now allows you to update your contact information online. We strongly recommend updating your contact information using your My.SSS account instead of submitting the E4 form. However, if you are requesting other changes (e.g. correction of name), you can update your contact information at the same time.

G. Updating of Bank Information

If you want to enroll your bank account with the SSS, check the box that corresponds to the intended purpose, and write the name of the bank, branch and account number. You can also enroll your bank account on your My.SSS account.

  • PESO Fund – The Personal Equity and Savings Option (PESO) Fund is a voluntary provident fund that is exclusive to SSS members.

H. Updating of Member Record Status

Temporary SSS members can upgrade their membership status to permanent by checking this box and indicating their submitted documents, a list of which can be found on the SSS E4 form.

I. Updating of Dependent(s)/Beneficiary(ies)

Accomplish this section if you want to add or remove dependents or beneficiaries. Write their name, relationship to you, and date of birth, and then check either “New/Additional” or “Deletion.”

Certification

The last step is to sign the certification stipulating that the information you provided are true and correct. Write your 10-digit SSS number and then write your name, signature and date.

If you or the member cannot sign, the SSS receiving personnel will assist in affixing the fingerprints. For E4 forms filed by the employer, include the names, signatures, addresses and contact numbers of two witnesses.

How to Submit Your SSS E4 Form

After filling out the E4 form in two (2) copies, submit it to the nearest SSS branch office together with the required documents. Check out the “List of Documentary Requirements” section of the E4 form for the complete list of required documents.

Don’t forget to bring your Unified Multi-Purpose ID (UMID) or Social Security (SS) card. In the absence of a UMID or SS card, bring two (2) valid ID cards with signature and photo.

How to Request a Copy of Your SSS E4 Form Online

Do you need a copy of your SSS E4 form for personal, travel, employment or business purposes? You can simply request a copy of your E4 form online using your My.SSS account, and then SSS will send it to you via email.

Take note, however, that although requesting a copy of your SSS E4 form only takes a few minutes, you will have to wait several months before you will receive it. That’s because it takes time for the SSS personnel to manually locate your records and send it to you.

Another thing to consider is that you can request for your E4 form only once. So only make this request if it’s really important. After you’ve received your copy of your SSS E4 form, keep it in a safe and secure place such as Google Drive or Dropbox.

Step 2: After logging in, click “E-SERVICES” on the navigation menu.

Step 4: You will be shown a list of SSS records that you can request. Check the box for “Member’s Data Amendment Form (E-4)” and then click “Submit.”

A popup dialog will appear with this message: “This action will send the supplied information to SSS. Do you want to continue?” Click “OK.”

Step 5: You have successfully submitted your request for your SSS E4 form. You will receive a confirmation email with your transaction number and other pertinent details. Don’t delete this email as you might need the transaction number to follow up the status of your request with the SSS.

How to Check the Status of Your Request

You can check the status of your E4 form request in your My.SSS account. To view the status of your request, go back to the Request Records page by clicking “E-SERVICES” and then “Request Records.” You will see a list of your previous requests along with their request date and email date.

If your E4 form request is still being processed, you will see the status “Under Process” on the “Email Date” column.

How Long Does it Take to Receive Your SSS E4 Form?

As previously mentioned, it normally takes several months to receive a copy of your SSS E4 form. The reason for the long waiting time is because the SSS will have to find your records, convert it to digital format, and then send it your email. SSS receives and processes thousands of these requests each day which takes significant amount of time and manpower.

That’s why you should only request a copy of your E4 form if it’s really important but not urgent, since you can request for it only once and it takes ages to arrive. Regularly check the status of your request by logging in to your My.SSS account.

Once you’ve received your E4 form, don’t forget to save it to your computer or mobile device. You can also upload it to Google Drive or Dropbox as a backup, just in case you accidentally deleted it from your device. If you have a printer, you can print several copies of your E4 form and keep the copies in a safe and secure place.
